A membership certificate issued by Canton Lodge, No. 498, of Canton, Pennsylvania, to O. L. Larcomb. Dated February 10, 1878. Imprint on bottom reads, "W. J. Morgan & Co Lith Cleveland Ohio." Founded in Utica, New York, in 1851, the Independent Order of Good Templars is still in existence and has changed its name to the International Order of Good Templars.

Williams' date of birth is recorded as October 10, 1866 and a patent date of 1904 is printed on the inside cover.

On the reverse of this application, Minnehaha Lodge, No. 95, provides a brief article (I.O.G.T. stands for International Order of Good Templars) that defines its parent organization and its mission. Imprint at the bottom of the article reads, "J.W. Dempsey Printing Co., Inc. Elmira, N.Y."
