Engraved Masonic symbols including double headed eagle, Corinthian column, globe, linked chain surrounding text. Within each link a symbol of the 33 degrees is depicted. In the lower right and left corners, there are seals of the Massachusetts Consistory and the Supreme Council of the Northern Masonic Jurisdiction.

The document has gold foil seal in the lower left corner. It is signed by Charles H. Spilman, Grand Secretary-General and William M. Mask, Commander-in-Chief, Delaware Consistory.
