On the reverse of this application, Minnehaha Lodge, No. 95, provides a brief article (I.O.G.T. stands for International Order of Good Templars) that defines its parent organization and its mission. Imprint at the bottom of the article reads, "J.W. Dempsey Printing Co., Inc. Elmira, N.Y."
