Black and white lithograph of the Knights of Pythias' emblem. A knight's helmet on top of a shield with the letters "F.C.B" on the inside. Two axes are on either side of the helmet. Inside the shield is the motto "esto perpetua."

The ledger is divided into Alphabet and Black Ball Roster. Black Ball Roster lists men by date and gives reason for their dismissal, for example "N.P.D" (probably unpaid dues), drunkeness, improper conduct, unknightly conduct, etc.
